Player Waiver and Release

Every player must complete this before participating in any Clutch Elite Basketball activity.

1. Assumption of Risk

I understand that basketball is a physical contact sport that involves inherent risks of injury, including serious injury or death. I voluntarily assume all risks associated with participation in Clutch Elite Basketball games, practices, and events.

2. Release of Liability

I release and hold harmless Clutch Elite Basketball, its owners, officers, staff, officials, volunteers, sponsors, and any host facility from all claims, demands, or causes of action arising from my participation, to the fullest extent permitted by Georgia law.

3. Medical Authorization

I authorize league staff to seek emergency medical treatment on my behalf if I am unable to consent, and I accept financial responsibility for any medical care provided.

4. Code of Conduct

I agree to conduct myself with sportsmanship and respect toward players, officials, staff, and facility personnel. Fighting, threats, harassment, or abuse of officials may result in suspension or removal without refund.

5. Photo and Video Release

I grant Clutch Elite Basketball permission to use photographs, video, and game footage in which I appear for promotional and league purposes without compensation.

6. Facility Rules

I agree to follow all rules of any facility hosting league activity, including parking, entry, conduct, and cleanup requirements.

7. Age Certification

I certify that I am at least 30 years of age before the first scheduled game of the season and that I will provide valid photo identification upon request.

8. Acknowledgment of League Rules

I acknowledge that I have read and agree to abide by all Clutch Elite Basketball league rules, roster rules, and eligibility requirements.

9. Electronic Signature Consent

I agree that typing my name below constitutes a legally binding electronic signature with the same effect as a handwritten signature.
